Essential 8™ Assessment Tool Completion page
This American Heart Association's Life's Essential 8™ online Assessment Tool page is part of the My Life Check® assessment and says there are 18 simple questions that take approximately four minutes to complete. It says "Make sure your heart health score is current to most accurately reflect your health". On this examplem the profile has been completed and that the person has answered the questions on health behaviors, health factors and well-being sections and has accessed their heart health score.
